What this site is — and is not

This site is an index. It reproduces form numbers, titles, categories, and revision dates verbatim from official government indexes and links to the official pages and PDFs. It does not host or mirror form files, offer drafted templates, provide instructions for completing any form, or give legal or tax advice of any kind. If you need advice, consult a licensed attorney or tax professional.

Federal court forms (304 entries): the national forms index at uscourts.gov/forms-rules/forms, published by the Administrative Office of the U.S. Courts. Because uscourts.gov was intermittently unavailable when this index was compiled (July 2026), the listing was captured from the Internet Archive’s June 17, 2026 snapshot of that page; all links on this site point to the live uscourts.gov pages.
